  • Q. Your overall impressions of this provider?
    A. On 7/28/07 I was discharged from a Skilled Nursing Facility. Occupational Therapy and Physical Therapy were ordered by my doctor and by the Skilled Nursing Facility. The Occupational Therapist responded within a few days. After three weeks there has been no word from a Physical Therapist. Attempts to find out why not have been unsuccessful. A primary reason to order home care Physical Therapy was to continue progress having been made while in the Skilled Nursing Facility. After three weeks I feel that this window of opportunity has been lost. I also feel that my physical condition, mainly strength and stamina, have diminished since being discharged from the Skilled Nursing Facility.
