Intercommunity Action, Inc. is a nonprofit community corporation that services three divisions: aging, developmental disabilities and behavioral health. The aging division is composed of an adult day services center, a community center for adults fifty-five and older, geriatric counseling services and the Share-the-Care program. This program offers affordable home-based services after an in-depth professional assessment. A social worker makes the initial visit to create an individualized care plan and to match the client with an appropriate caregiver. The same social worker then monitors the plan's implementation and maintains a close relationship with the caregiver to assure quality service. Referrals are provided as needed.
