ElderServe Home Care is a nonprofit affiliate of Hebrew Home for the Aged at Riverdale, a multi-specialized provider of long-term care for seniors. Clients benefit from a wide range of health and social services, including physical, occupational and speech therapies, personal grooming, housekeeping, home maintenance, nursing services and medical management. The case manager, who is also a nurse, organizes and oversees clients' individual care plans. Case managers communicate with clients' families, as well as their physicians, to ensure that each client receives the health care that best meets their unique needs.
