All Matrix AdvoCare Network care management is done by registered nurses who are at the top of their profession. Our RN Care Mangers conduct comprehensive assessments of the myriad needs of each client. They work together with the client, family, physician, and other interested parties to develop creative, responsive care plans designed to maximize each client's functional status, independence, and dignity. Matrix can provide home care services if requested, and regularly refers clients to other community resources and living environments if staying home is not an option. And, since Matrix Care Mangers make recommendations, not decisions, they work cooperatively with health care providers and you to develop and implement an appropriate plan of care.
