Fox Hill Village Assisted Living Program (Is this your business?)

It is the intent of the Assisted Care Department and Fox Hill Village to provide the highest quality of personal care and services in an environment that maximizes and ensures the dignity and respect for the individual resident. Under Full Assisted Care Services, the resident is entitled to 1.5 hours of Home Health Aide assistance and supervision by the nursing staff per day. There is an additional charge for time spent over the allotted 10.5 hours per week.

Home Health Aide activities should reflect the need for assistance with two or fewer activities of daily living.

1. Mobility:
   • Wheelchair transport.
   • Accompany residents to and from Fox Hill Village activities, meals, in-house appointments, etc.
   • Guidance for visually impaired residents.
   • Ambulation as part of a daily exercise regime.
   • Transfers w/ minimal assist of no more than one.
2. Personal Care:
   • Assist with bathing, showering, shaving, shampooing, mouth care, and other aspects of personal hygiene.
3. Toileting:
   • Assist with ordering supplies for bladder incontinence. Resident must be able to manage incontinence independently
4. Dressing:
   • Assist with dressing and undressing including TED stockings
5. Meals:
   • Menu review & pick up/ drop off
   • White bag pick up or pick up from the grill
   • Assist with light meal prep

Performance of Manual Tasks:

• Includes light housekeeping, bed making, trash removal, loading and/or unloading of the dishwasher, hand washing dishes, laundry, bathroom touch-up, watering plants, pick up mail.

Medication Management (“Med-on-Time” program)

Services included:

• Send current medication list to MD appointments. Resident responsible for informing ACS of any scheduled appointments
• Monthly medication card exchange including any additional card exchanges needed during the month
• Communication with pharmacy and/or MD office as needed
• Reordering medications
• Medications changes, new medication orders
• Coumadin management including labs
• Visiting nurse communication as needed
• Eye drops, ear drops, topicals
• Dental premedication

*All medication prompts are done by assisted care service certified nursing assistants. Private aides are not permitted to provide medication prompts.

CERTIFIED NURSING ASSISTANT services available for additional fees

Outside Accompaniment: Late morning until early afternoon as schedule allows. Off-site trips to physicians' offices, social activities or any other venue. This would be scheduled through the department, as possible. Other: Any other service not normally related to personal care and normal activities of daily living, if and as agreed to, by the department supervisor.
