"The Pace program is a licensed medical adult day health that provides complete care management for people who are nursing home eligible but are living in the community. The center provides personal care, medical care, medication monitoring,one meal a day and transportation. Enrollees must attend the day care at least once a week and agree to receive all of their medical care within the PACE system."
