ASI is an independent, for-profit company that has been providing Client-Centered Care Management to elder and disabled adults. Client-centered management (CCCM) is a proven, comprehensive client-focused collaboration that includes education, provider coordination, and client advocacy. It emphasizes the selection and coordination of services from the client's perspective and considers all of the client's circumstances.

ASI's finely tuned CCCM model has the ability to embrace a client that is experiencing difficulty with issues affecting their health, mental well-being and integration within the community. By working side by side with clients and/or family members ASI is able to establish a Care Management Plan that follows more of social model than a medical model.

Each client in the CCCM model has a complex-care team consisting of

• a Nurse Care Manager, who provides on-site assessments, and is available to contact the patient's treating physician to clarify care issues and troubleshoot in support of the treating physician's plan
• a non-clinical care coordinator, who coordinates team reviews and carries out all coordination efforts that do not require the skills of the nurse and may also act as a liaison with the health plan;
• the client's physician, who provides the team with information on the medical care of the client, and in collaboration with the care manager, anticipates the patient's medical problems.
