Bay Area GCM Description

Founded in 2000, Bay Area GCM's professional care management & therapeutic services are devoted to trusting and respectful relationships with older adults and their families.

Careful attention is given to: assessing strengths and needs of older adults; matching caregivers to meet their needs; supporting the ability to remain at home or transition into community living; improving communications between health care providers, older adults, and family members; and providing therapeutic services. Therapeutic activities are designed to meet older adult's emotional, creative, intellectual, relational, and physical needs.

The experience, education, training, and volunteer commitments of the team, include extensive work with all levels of memory loss, progressive degenerative and chronic diseases, and end-of-life care.

To ensure quality of care, each professional geriatric care manager works with fewer than 6 clients.

Owner Sharon Gray is a licensed Marriage & Family Therapist and care manager certified with a certificate in gerontology.
