It is Integrative because it involves the whole person and ways to maintain and improve their quality of life, with the understanding that "improvement" doesn't always mean a return to one's previous state. Rather, it may mean making the best of the state that now exists for the person.

Our care management focuses on realistic outcomes that are both achievable and measurable. Clients and their families know what to expect, and can readily judge the progress we make along the path of servicing the care needs of the individual. We work to develop a plan of care for our clients and their families to foster independence for as long as possible and, when necessary, assist with finding appropriate solutions to long-term care.

Late Life Journey care managers work with each client to prioritize needs, work within one's economic abilities, and helps to arrange, monitor, and manage the unique care needs of each individual.
